Job Title & Role Description:

The Patient Care Specialist plays a vital role in delivering chronic care management for patients with complex health needs. Primarily working outside of clinical settings, this position entails conducting home visits (90% of the time) and engaging in virtual care options (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, setting up virtual appointments with healthcare providers, and ensuring adherence to care plans. Through the development of strong relationships, effective care coordination, and patient education, the Patient Care Specialist contributes significantly to improving health outcomes and minimizing hospitalizations.

Skills Required:

  • At least 2 years of relevant work experience, such as in roles like Community Health Worker, Peer Support Specialist, or Medical Assistant.
  • A high school diploma or GED is mandatory.
  • A valid driver's license, auto liability insurance, and reliable transportation for patient outreach.
  • Prior experience in chronic care management or working with elderly/chronically ill patients is essential.
  • Proficient in using technology, including basic computer skills and electronic medical records (EMR) systems.
  • A background in motivational interviewing and trauma-informed care is preferred.
  • Excellent interpersonal skills to effectively communicate with patients and team members.
  • Strong organizational abilities, with the capacity to manage schedules independently in flexible environments.
  • Multilingual skills are a bonus but not required.
  • Previous experience in home care settings is advantageous.
